Overview of Florida SMMC Long-Term Care
Florida's Statewide Medicaid Managed Care Long-Term Care (SMMC LTC) program provides home and community-based services and nursing-facility care to eligible adults 18 and older. AHCA manages contracts and enrollment, the Department of Elder Affairs (DOEA) through CARES assesses medical eligibility and level of care, and the Department of Children and Families (DCF) determines financial eligibility. As of February 1, 2025, SMMC 3.0 organizes the state into nine regions.
